First things first. A consultation is done with the client and/or family to assess the client's physical status and psychosocial needs. Based on the level of intervention required, recommendations are made and an integrated plan of action is developed.
Support Services
The Care Manager facilitates the plan of care and acts on the client/family's behalf, as an advocate and liaison with the medical community. We also assist with Durable Power of Attorney for Health Care Agents.
Suzanne Slaughter is also Court certified with the Superior Court of San Diego to act as Conservator of Person for Health Care. She can help in directing you to the appropriate financial agents i.e. Trust Officers, Private Fiduciaries, Conservators of Estate and Successor Trustees.
